Reporting a safety concern as staff
How to use Report incident to log a safety, behaviour or wellbeing concern about a child, and what happens next.

Why this exists
When something worries you about a child, you should not have to work out who to tell or where to write it down. Report incident is on every staff screen, asks a few short questions and sends your report straight to your centre's safety lead. You can act in the moment and know it reached the right person.
If anything worries you about a child's safety, tell your centre straight away. Every staff member can file a report from any screen.
If a child is in danger now
Call 000 first. In NSW you can also call the Child Protection Helpline on 132 111. Then file the report for the records.
Filing a report
- Press Report incident in the header. On a phone it is the shield button.
- Answer the short triage questions. They help the centre's safety lead decide how fast to act.
- Choose a category and, if it is about a particular child, choose the student. A tutor sees the students they have taught; admins and compliance staff see every student at the centre.
- Describe what you saw or heard. Stick to the facts and use the person's own words where you can.
- If you said or did something in the moment, note it.
- Press Submit incident.
If you opened the form from a lesson, the lesson is filled in for you.
What happens next
- The report goes to your centre's safety queue. Urgent reports alert your centre's compliance team in the app straight away.
- Your centre's safety lead decides every time what, if anything, the family is told. Do not contact the family about the report yourself unless the safety lead asks you to.
- Admins and compliance staff work the reports under Safety > Moderation queue and Safety > Incidents.
Good to know
- Reports are kept. Nothing you file is deleted when a child leaves.
- Training courses may ask you to file a practice report. A practice report is recorded as training only and never reaches the safety queue.
